DEPOSIT, CANCELLATION/REFUND/NO-SHOW AND RESCHEDULING POLICIES

• PAYMENT and DEPOSIT: A 25% deposit of the total estimated service cost is required at the time of booking to secure your desired dates and nursing team. The remaining balance is due 7 days prior to the scheduled start of services. For bookings made less than 7 days within the service start date, the full payment is required at the time of booking. Our agency accepts payment in the form of a credit card, debit card or direct withdrawal from a bank account.

• CANCELLATIONS AND REFUNDS: By Client: 7 days or more prior to service start date: The initial 25% deposit is refundable, minus a 10% administrative fee. Any additional payments made will be refunded, minus a 10% administrative fee. For cancellations less than 7 days prior to service start date: The initial 25% deposit is nonrefundable and will be retained by LCC to cover incurred costs and lost booking opportunities. Any additional payments made will be refunded, minus a 25% administrative fee. We encourage clients to

confirm their surgical date and recovery needs carefully before booking. Cancellation by LCC: In the rare event that LCC must cancel services due to unforeseen circumstances, a full refund of all payments made will be issued. We will endeavor to provide as much notice as possible and assist in finding alternative reputable care providers if desired.

• RESCHEDULING: By Client: 14 days or more prior to service start date: Clients may reschedule their services once without penalty, subject to nurse availability. Any subsequent reschedules may incur a $250 rescheduling fee. New service dates must be within 3 months of the original service start date. Between 7-13 days prior to service start date, rescheduling is subject to nurse availability and will incur a flat rescheduling fee of $500. New service dates must be within 3 months of the original service start date. Less than 7 days, rescheduling is generally not permitted due to the immediate impact on our nursing team's schedule and other potential client bookings. In exceptional circumstances, and at the sole discretion of LCC, a rescheduling may be allowed and will incur a 25% of total service cost rescheduling fee. We will do our best to accommodate surgery date changes.

• NO SHOWS: If a client fails to be present or accessible at the agreed upon time for scheduled services without prior notification, it will be considered a no-show, and no refund will be issued for that service. Rescheduling of the missed service is at the discretion of the Administrator.
